Dear
Parent(s)/Guardians(s):
New Jersey State Law requires that all students must be
immunized against certain preventable diseases. Students, who do not present
proper documentation prior to school starting in August 2008, will be excluded
from school.
For students, born after January 1, 1997 and who will be in Grade Six in
August 2008
- N.J.A.C.
8:57-4.10 requires children born after January 1, 1997 and enrolled in
Grade Six or transferring into a
New Jersey school from another state or
country to receive one dose of the tetanus, diphtheria, acellular pertussis (Tdap) vaccine.
- N.J.A.C.
8:57-4.20 requires children born after January 1, 1997 and enrolled in
Grade Six or transferring into a New Jersey school from another state or
country to receive one dose of meningococcal (meningitis) vaccine.
For
students enrolled in a Preschool Class
- N.J.A.C.
8:57-4.18 requires children attending child care centers and preschools that
are two to 59 months of age to receive the pneumococcal conjugate
vaccine series.
- N.J.A.C 8:57-4.19 requires children attending child care
centers and preschools that are six to 59 months of age to receive one annual dose of influenza
(flu) vaccine.
** All previous immunizations requirements are still in effect.
